No more than 2 Out of Service Violations in the last year None in the past 3 years: Leaving the scene of an accident. Causing a fatality through negligent operations of a CMV. Driving while CDL is suspended, revoked, or canceled. Railroad-Highway Grade crossing violations. Violating out of service orders. No violations in the past 12 months and no more than one violation in the past 3 years: pre CDL majors & multiples will be reviewed. Speeding 15mph or more Careless and/or Reckless driving Following too close Improper or erratic lane changes Driving a CMV without obtaining a CDL Driving a CMV without a CDL in possession Driving a CMV without the proper class or endorsements for cargo being hauled Texting and/or handheld mobile violation Speeding in a marked construction or school zone No DUI/DWI’s in the last 5 years Automatic DQ’s: recent at fault rollover in the past year & No failed D.O.T alcohol/drug test lifetime
